• In column 1, the date and time of the incident are mentioned.
• In column 2, the name and address of the complainant
• Column 3 contains a brief description of the crime
• Column 4 mentions the place of occurrence and the direction from the police station.
• If there is any delay in filing the F.I.R., the reason for it is mentioned in column 5.
• The date and time of departure from the police station are mentioned in column 6.
